POSITION: MEDICAL SOCIAL WORKER
RESPONSIBLE TO: Social Work Supervisor, Hong Fook Center
COMMITMENT (HOURS): 40 hours per week
STATUS: Non-Exempt (3 months orientation period)

JOB SUMMARY: Under the supervision of the Social Work Supervisor, the Medical Social Worker is responsible for psychosocial assessment and case management services at the Hong Fook Community Based Adult Services. This will include screening potential participants, completing intake assessments, providing psychosocial assessment to participants, developing care plans, and working effectively with a multidisciplinary team to develop appropriate treatment plans.

JOB SUMMARY: Under the supervision of the Social Work Supervisor, Social Worker Assistant will provide social work services to the participants of Hong Fook Centers. This will include intake of prospective participant, provide language assistance services, work with other team members to provide care management services to the participants, and assist the Social Work Supervisor in the psycho-social assessment of the participants.
