DESCRIPTION

The City of Long Beach, Department of Health and Human Services is recruiting for a full-time Public Health Professional II to serve as the Homeless Services Resource Coordinator in the Human Services Bureau. Reporting to the Homeless Services Officer, this position will work to expand, coordinate, and deploy City-wide coverage of outreach services to those experiencing homelessness. This position is located at the Multi-Service Center, 1301 West 12th Street, Long Beach, CA 90813.

EXAMPLES OF DUTIES

  • Prepares Homeless Management Information System evaluations and reports;
  • Supervises direct line staff and assists in the operations of the Multi-Service Center;
  • Coordinates with City Departments and external agencies to establish best practices for outreach services and develop tools to assess the effectiveness of outreach;
  • Serves as a liaison to homeless services providers and City first responders to identify areas of need and coordinate outreach rapid response;
  • Oversees the City of Long Beach, Homeless Services Division's outreach hotline and educates community members on outreach services;
  • Facilitates the Discharge Planning Collaborative, a congregation of hospitals located within the City to facility discharge planning processes and social services;
  • Establishes and maintains working relationships with existing and new partners to streamline referral linkages to mental health care, health services, and other supportive services;
  • Implements Coordinated Entry policies and procedures among the Outreach Workgroup;
  • Works closely with Coordinated Entry staff to streamline vulnerable individuals into available housing opportunities;
  • Identifies areas of improvement in the existing outreach response system and creates effective solutions to address and remove barriers to streamlined responses;
  • Prepares reports to track accomplishment of strategic goals and evaluates work performed;
  • Conducts community engagement activities around homeless services to increase access to services for special needs populations;
  • Oversees the Homeless Services Division Social Media presence and increase awareness of services available to the Long Beach community;
  • Assists and supports crisis response and triage efforts; and,
  • Performs other related duties as required.

REQUIREMENTS TO FILE

  • Bachelor's Degree in Social Work, Public Health, Public Policy, Public Administration, or a closely related field. Copy of degree or transcripts denoting degree conferment required. Professional experience supporting outreach activities in a public health program beyond the minimum requirement may be substituted for required experience on a year-for-year basis.
  • One year of experience supporting outreach activities in a public health program.
  • Valid motor vehicle operator license. (Required to obtain a California motor vehicle operator license within 6 months if out of state).
DESIRABLE QUALIFICATIONS:
  • Master's Degree in Social Work, Public Health, Public Policy, Public Administration, or a closely related field. Copy of degree or transcripts denoting degree conferment required.
  • Professional experience supporting homeless services programs.

  • Excellent interpersonal, written, and verbal communication skills.
  • Ability to deescalate and resolve conflict effectively.
  • Ability to work independently with general supervision and exercise sound judgment.
  • Ability to work within deadlines, manage multiple assignments, and set appropriate priorities.
  • Proficiency in the use of computers, including email, internet, and office productivity software (e.g. Microsoft Word, Excel, and PowerPoint).
